Preparing for Your Visit
Your Treatment Plan
Insurance information patients should know
Before your first appointment, please provide your current insurance information through your intake forms or contact us with a clear photo of the front and back of your insurance card. We will do our best to verify your benefits before treatment, but verification is not a guarantee of payment by your insurance company.
We are in-network with Blue Cross Blue Shield, Allegiance, Pacific Source, VA-Triwest, Cigna, and Aetna. For other insurances, we can provide a superbill for potential reimbursement. Please verify your acupuncture benefits before your first visit to help prevent unexpected costs.
What your plan may include
Your acupuncture benefits may depend on your specific plan and can vary based on:
-
Whether acupuncture is a covered benefit
-
The number of visits allowed each year
-
Your deductible, copay, or coinsurance
-
Whether a referral or prior authorization is required
-
Whether coverage applies only to certain diagnoses or providers
-
Whether your plan considers our clinic in-network or out-of-network
Your financial responsibility
You are responsible for all charges not paid by your insurance plan, including deductibles, copays, coinsurance, non-covered services, missed appointments, and balances resulting from denied claims.
If benefits cannot be verified before your visit, payment may be due at the time of service. If your insurance later pays for covered care, we will apply that payment to your account according to your plan benefits.
Questions about coverage?
The most accurate source for your benefits is your insurance company. Call the member-services number on the back of your card and ask specifically about acupuncture coverage, visit limits, deductible status, copay or coinsurance, referral requirements, prior authorization, and whether Missoula Acupuncture is covered under your plan.
We are always glad to help explain the information you receive and answer questions about our office policies.
