Verify Your Benefits Before Your Visit
Insurance plans vary widely, and acupuncture coverage is determined by your individual plan—not by Missoula Acupuncture. Before your first appointment, please call the member-services number on the back of your insurance card to confirm whether your plan covers acupuncture for your reason for seeking care.
Coverage may depend on your plan, diagnosis, provider network status, deductible, copay or coinsurance, referral requirements, prior authorization, and annual visit limits. A benefit quote is helpful, but it is not a guarantee of payment. Your insurance company makes the final determination after it processes your claim.
What to Ask Your Insurance Company
When you call, please ask:

Your Financial Responsibility
Patients are responsible for copays, coinsurance, deductibles, non-covered services, visits beyond plan limits, and any balance not paid by insurance. Please notify us before your appointment if your insurance plan, employer, member ID, referral, or authorization changes.
When applicable, we can submit claims to participating insurance plans and help you understand the benefit information available to us. If we are out of network with your plan, we can provide a superbill for you to submit to your insurer for possible reimbursement.
The explanation above aligns with your existing office policy: coverage is determined by the insurer, benefit checks do not guarantee payment, and the patient remains responsible for amounts their plan does not pay.
